This is your opportunity to update your child’s vaccines.
18 years and under WITH insurance:
If you have health insurance (EXCLUDING Medicaid), please print and fill out forms from the previous page and bring them and your insurance cards with you to the event.
18 years and under WITHOUT insurance or have Medicaid:
If you do NOT have insurance or do have Medicaid, please print and fill out forms from the previous page and bring them, and your Medicaid card if you have Medicaid, with you to the event.
19 years and over WITH any insurance (INCLUDING Medicaid):
If you have health insurance (INCLUDING Medicaid), please print and fill out forms from the previous page and bring them and your insurance cards with you to the event.
19 years and older WITHOUT insurance:
Please fill out forms from the previous page and bring them and your ID with you. (Limited vaccine types)
